TO BECOME A MEMBER OF AN EXISTING MULTI-STATE PROJECT A FACULTY MEMBER MUST COMPLETE THE FOLLOWING FORMS AS DIRECTED:

1) APPENDIX E (complete electronic version and email as an MS Word Attachment to T. Simmons. If your computer system does not allow you to complete the electronic version please complete the attached paper version and mail to T.Simmons, Rm. 1206 Symons Hall.
